DirectDx Affiliate Program Terms of Service

Effective Date: 2-16-2020

1. Agreement Acceptance

By joining the DirectDx Affiliate Program, you agree to abide by the Terms of Service outlined herein.

2. Program Participation

The Affiliate Program is intended for individuals who own or are licensed to use the entire contents of their website and ensure that their website and marketing efforts comply with all applicable laws and regulations, including but not limited to copyright, trademark, patent, and other proprietary rights.

3. Prohibited Activities

Affiliates are prohibited from using DirectDx marketing materials for spamming purposes, promoting our services through paid ads without permission, and using DirectDx’s logos, images, company name, personnel names, or images without express permission. Affiliates must not promote our services to healthcare practitioners not licensed to order lab tests in their jurisdiction.

4. Commissions

To qualify for commissions, the referred physician must either sign up with your unique affiliate link or submit your name during the signup process. Affiliates will receive a commission of 3% of the total order value for Quest lab tests only (minute the Quest draw fee). Commissions for any other laboratories are not applicable. Commission amounts are subject to adjustment in cases of refunds. Payments are made monthly on the 15th for totals exceeding $100. Affiliates earning over $600 annually will receive a 1099 form and are responsible for reporting their income for tax purposes.

5. Payment Method

Affiliates give DirectDx consent to send payments through PayPal, Venmo, or Zelle.

6. Termination

DirectDx reserves the right to terminate the affiliate relationship at any point for any reason. Upon termination, outstanding commissions will be paid, after which no further payments will be made.

7. Liability and Indemnification

Affiliates agree to indemnify and hold harmless DirectDx from any claims, damages, or expenses arising from their misuse of the Affiliate Program.

8. Amendments

DirectDx reserves the right to modify these terms at any time. Continued participation in the Affiliate Program following such modifications constitutes acceptance of the new terms.

9. Governing Law

This agreement shall be governed by and construed in accordance with the laws of the jurisdiction in which DirectDx operates.
